Private Duty Nursing Service Request Information

When a student's health needs require immediate or intensive skilled one-to-one nursing care, the parent/guardian may request the student's private duty nurse (hired by the parent/guardian) attend school with the student. 

To request that a private duty nurse attend a Keller ISD campus with a student all documents in the packet below need to be completed and submitted for approval.

When the Private Duty Nurse Request Packet has been completed, all documents need to be submitted through the Google Form link below.  Please allow 10 business days for full consideration of the individual case.  Final decisions will come by e-mail to parent and campus administration.
