Physical-Medical History Form - -Must have a physical dated after January 1, 2021 for the 2021-2022 school year.  Physicals are DUE to TSMS by Aug. 17th, 2021.

ORDERS CLOSE JULY 25TH TO ENSURE FIRST WEEK OF SCHOOL DELIVERY.

This means that the athletes will be allowed to wear “whatever they got” for athletics until this order of uniforms comes inSo whatever athletic shorts (no jean shorts/skirts/leggings) and t-shirts (no v-neck or tank tops) that they already have, will be acceptable.  Once all uniforms come in and are passed out, the girls will then be expected to be in either this TSMS athletic gear OR their own black shorts and plain grey t-shirt.

 Please wear your own BLACK shorts and GREY t-shirt if you do not purchase our options above.
