Private Duty Nursing Services
When a student's health needs require immediate or intensive skilled one-to-one nursing care, the parent/guardian may request the student's private duty nurse (hired by the parent/guardian) attend school with the student.
Instructions for Private Duty Nurses For a Private Duty Nurse to attend a Keller ISD with a student, prospective providers must complete both of the following requirements before they can be approved:
1. Online Application All new Private Duty Nurses are required to complete the Health Services Service Provider Application found on the Keller ISD job opportunity page.

2. Request Packet and Submission In addition to the online application, you must complete the full Private Duty Nurse Request Packet. Once all documents in the packet are finished, they must be uploaded via the Google Form linked below.
