Immunizations

Texas school and child-care facility immunization requirements are determined by the state legislature and set by the Texas Department of State Health Services, in conjunction with the Texas Education Agency.
Students must be current with immunizations to attend school unless an exemption is on file with the school according to Texas Education Code. 

Immunization records will be accepted if they include the student's name, student's date of birth, physician's or public health personnel's signature, initials or stamp.
