Students planning to participate in UIL athletics in grades 7-12 for the 2010-11 academic year must fill out the required extracurricular paperwork to be eligible.
Parents and students can now download the UIL Extracurricular Packet themselves and print it off at home.

Who has to have UIL athletic paperwork?
The University Interscholastic League (UIL) requires a medical history be done each year on all student-athletes participating in sports.
Physicals are required by UIL for student-athletes currently in 6th grade going into 7th, 8th graders entering 9th grade, and 10th graders entering 11th. If you are new to Keller ISD, proof of a physical is required regardless of what grade the new student is entering.
What forms does the UIL require?
UIL requires the following annual forms be kept on file by a school for each student-athletes:

Pre-participation Physical Examination

Medical History Form

Parent or Guardian Permit (titled Emergency Card in KISD's packet)

Rules Acknowledgement

Parent/Student Illegal Steroid Use Form
Can a student participate in a sport without completing the UIL forms?
No. All of the paperwork must be completed before a student-athlete can even practice with his or her team.
Where do I turn in the UIL Extracurricular Packet?
Once completed, the packet should be returned by the end of the school year (June 4, 2010) to the coach or physical education teacher on the student's campus.
- Current 6th Graders - turn in to the physical education teacher at your current campus
- Current Middle School student-athlete - turn in to the current middle school coaches
- Current High School student-athlete - turn in to head coach for which you played for in the present school year. If you are a multi-sport athlete, return it to the head coach of the sport that occurs first in in the 2010-11 school year (Ex: If you play volleyball and softball, turn the paperwork in to your volleyball coach as the volleyball season takes place prior to softball in the 2010-11 school year.)
